Colorado Medicaid enrollment is processed by the Department of Health Care Policy and Financing through its fiscal agent, using the online Provider Web Portal built on the interChange system. You start an application that supplies your NPI, your active Colorado professional license, your taxonomy, your service and pay-to addresses, your electronic funds transfer details, and your ownership and managing-control disclosures, and you attest to the provider participation agreement. The state screens the application against the federal CMS risk level assigned to your provider type and verifies you against the OIG exclusion list, the federal SAM database, and Colorado exclusion records.

Most Health First Colorado members get their care coordinated through the Accountable Care Collaborative, in which the state contracts with Regional Accountable Entities, the RAEs, across seven regions. Each RAE manages a regional network for physical and behavioral health. Enrolling with Health First Colorado through the Provider Web Portal is the foundation, but to participate fully and receive referrals you generally also need to join the network of the RAE that covers the region where you practice.

Every Medicaid provider is assigned a federal risk level of limited, moderate, or high based on provider type, and Colorado screens accordingly. Limited risk involves licensure and exclusion checks. Moderate risk can add an unannounced pre-enrollment or post-enrollment site visit. High risk can require fingerprint-based criminal background checks for owners and managing employees. Certain institutional provider types also owe a federal application fee at enrollment and revalidation unless a hardship waiver or prior payment applies.
