Oklahoma SoonerCare Provider Enrollment, Handled End-to-End

Enrollment is processed by the Oklahoma Health Care Authority through its Medicaid management information system and the OHCA secure provider portal. You select a provider contract type, supply your NPI, your active Oklahoma professional license, your taxonomy, and your ownership and managing-control disclosures, then attest to the SoonerCare provider agreement. The state screens the application against the federal CMS risk level assigned to your provider type and verifies you against the OIG exclusion list, the federal SAM database, and Oklahoma exclusion records.

SoonerCare is strict about consistency. Your legal name, practice and pay-to address, and tax identification number must match across your NPI registration, your application, and your disclosures. We reconcile every data point before submission so OHCA has no reason to return your file for correction.

SoonerCare contracts providers differently depending on how you bill, and the wrong path is a common reason a file stalls. We complete the right contract for your situation:

• Individual providers who render and bill services under their own NPI.
• Group practices where each rendering provider holds an individual contract and is then linked to the group billing NPI and tax ID.
• Facilities and institutional providers such as clinics, hospitals, FQHCs, and agencies that contract as an organization.
• Ordering, referring, and prescribing providers who must be enrolled for SoonerCare claims to pay even when they do not bill directly.

For groups and facilities, we map your provider roster against your billing entities and confirm each link is active in the OHCA portal before claims go out.

Oklahoma has moved much of its Medicaid population into SoonerSelect, the managed-care program where contracted health plans coordinate medical, behavioral, and dental care for most SoonerCare members. A base SoonerCare contract with the Oklahoma Health Care Authority is the foundation, but to see SoonerSelect members and be paid for them, you generally also need to be credentialed and contracted with each plan that covers your patients.

We confirm whether your specialty and region require participation beyond base SoonerCare enrollment, then complete the SoonerSelect plan credentialing and contracting in the correct sequence so referrals and reimbursement flow without gaps.

Every Medicaid provider is assigned a federal risk level of limited, moderate, or high based on provider type, and Oklahoma screens accordingly. Limited risk involves licensure and exclusion checks. Moderate risk can add an unannounced pre-enrollment or post-enrollment site visit. High risk can require fingerprint-based criminal background checks for owners and managing employees, along with an application fee for certain institutional provider types.

Most delays are avoidable. The patterns we see most often are a practice or pay-to address that does not match your NPI record, a missing or expired Oklahoma license, incomplete ownership and managing-control disclosures, the wrong contract type or taxonomy, and an unanswered request for additional information from OHCA.

SoonerCare is rarely your only payer.
